The Fertility Test is an essential part of an individually tailored treatment. However, many people wonder if The Fertility Test is necessary if they have had treatment before or not.

YES

If you have had previous treatment, but these IVF treatments have been negative and/or have been performed at another clinic, the answer is YES.

REMEMBER, with a new complete review, things that have been overlooked always appear. These can be of crucial importance for the treatment outcome. Therefore, we would like you to bring the papers and test results from previous treatments. These will help us map out future treatment.

The next step is usually to take The Fertility Test.

The Fertility Test

The consultation lasts approx. two hours.

To start, you have a conversation with the doctor, enabling the doctor to form a picture of the problem and answer any questions.

The doctor does an ultrasound of the woman, and the lab examines the man's sperm sample.

The doctor assesses which treatment is best for you and sets up an individually tailored treatment proposal for you.

Furthermore, you get a conversation with a nurse who carefully goes through this treatment proposal. She informs you about all practical matters related to treatment (e.g. time aspect, ultrasound checks locally where you live, medication handling, injection instructions, prices, contract, etc.)

Requisition for standard blood tests such as HIV, Hepatitis and AMH, which must be taken before any IVF treatment is sent to you by post. You will take these blood samples at the nearest hospital at the first opportunity. Please note that these MUST be analyzed by a laboratory approved for infection testing for assisted reproduction. You will receive a list of approved laboratories. The test results will be sent to us and must be available to us before treatment can begin.

Common treatment protocols.

We offer many treatments at Klinikk Hausken. With us, you get an adapted IVF treatment. Although we customise the treatment plan for each couple, some elements will be similar. Below are four common examples of treatment protocols and expected costs.

IVF treatment 1

Short protocol is today standard treatment. Firstly the doctor makes an individual treatment protocol with detailed information about implementation. After that, the couple can choose which menstrual cycle they want to start treatment.

The woman starts with daily injections of hormones (stimulation) from menstrual day 2. You can choose between several medications. The doctor will guide you so you can find the one best suited to you.

From stimulation day 5, the woman will have to inject a drug called Orgalutran (to prevent ovulation) in addition to the hormone stimulation. This is also injected daily.

An ultrasound is planned around stimulation day 9. The ultrasound will be done by a collaborating gynaecologist near where you live. This survey will tell us how many eggs are maturing and which day will be the best for egg retrieval. When we have received the ultrasound finding, you will be informed about the further stimulation and when you will put the ovulation syringe (Ovitrelle). The day to retrieve the eggs is thus determined.

After the eggs have been extracted and placed in separate bowls, the man's sperm sample will be processed to achieve the best possible natural fertilization. In some cases, the semen sample will be reduced in number and motility. Then we do something called ICSI. We choose the best swimmers and inject the egg directly (which helps with fertilization).

After fertilization, the embryos are housed in an embryoscope (an incubator where further development happens).

2 days after egg retrieval is ready for the return of the embryo that has shown the best development. If you have several good embryos, these can be frozen for later return in the natural cycle.

From egg retrieval the woman will have to use vaginal capsules (Lutinus). These are inserted into the vagina. Lutinus is a hormone that helps the embryo attach and supports the early development of pregnancy. You continue with Lutinus until the pregnancy test. This is taken 14 days after egg retrieval.

IVF treatment 2

For some, it may be appropriate to use a long protocol. This means that you go through a pre-treatment (about 2-3 weeks) before you start with hormone stimulation. During pre-treatment the woman uses a nasal spay called Synarela. Treatment begins on menstrual day 20.

This protocol is often used if the female has reduced the number of eggs. The pre-treatment synchronizes the eggs and thus often produces more mature eggs than if one uses a short protocol.

Endometriosis patients will also benefit from using a long protocol. The down-regulation reduces the endometriosis activity and can be beneficial in order for the embryo to attach.

For those who want the treatment to occur in a specific week, one will also choose a long protocol. You only extend the nasal spray period so that you get the desired treatment week adapted. One is then not dependent on the menstrual rhythm of the woman.

After down-regulation with Synarela (2-3 weeks), the woman starts with daily injections of hormones (stimulation). Which day is agreed with us (treatment plan). Start-up day is, for most, a Wednesday. The woman continues with Synarela nasal spray also during the stimulation period.

An ultrasound is scheduled for Fridays, 10 days after the start of hormone stimulation. The ultrasound will be done by a collaborating gynaecologist near where you live. The ultrasound will tell us how many eggs are maturing and which day will be the best for egg retrieval. When we have received the ultrasound findings, you will be informed about the further stimulation and when you will put the ovulation syringe (Ovitrelle). Hormone stimulation/insemination

In cases where the woman lacks ovulation or has a maturation problem, it may be enough to give a hormone stimulation either with tablets (Pergotime or Femar), or with hormone injections as with IVF treatment (but smaller doses). If the man has normal sperm quality, the couple can have regular intercourse at home.

When we give hormone stimulation, we do an ultrasound on approximately stimulation day 10. If the treatment has been successful (mature eggs), the woman starts by examining LH in urine (Clear plan). When this has a positive effect, you have intercourse the same evening and the morning after. Sometimes we give an ovulation syringe (Ovitrelle) to be sure of the timing.

If the sperm sample is somewhat reduced, you can combine hormone stimulation with insemination. This is called the IUI. The insemination is done the day after positive LH, or timed with Ovitrelle. The sperm sample is optimized by removing sperm cells that are abnormal in shape and those that have reduced mobility. The best sperm are then concentrated and inserted into the uterine cavity with a catheter. This makes it easier for the sperm to fertilize the egg.

Insemination can also be administered in a natural cycle if all conditions in the woman are normal.

How much is one attempt?

Klinikk Hausken offers sperm donation. We use sperm from 2 large sperm banks in Denmark.

Insemination with donor sperm is called IUI Partner. This treatment is offered to heterosexual couples where the man does not have sperm production or where the sperm quality is so poor that one does not get good enough embryos that are able to give pregnancy. Sperm donation is also offered to gay (lesbian) couples.

After the couple has been to a pre-meeting and the necessary information about treatment has been given, the clinic makes the order from the sperm bank in Denmark. The donor must not be anonymous and is chosen according to the couple's characteristics (ethnicity, eye color and hair color). Semen is frozen in something we call straw. These are stored in our clinic pending treatment.

IUI Partner can be completed in a natural cycle and or stimulated cycle (hormone stimulation). If everything is normal with the woman, then one follows her natural cycle. An ultrasound examination is performed on menstrual day 10-11 to check that she is ovulating and that the mucous membrane in the uterine cavity is thick and fine. The woman then starts by examining LH in urine (Clear plan). When LH becomes positive, insemination is performed the next day. Sometimes we give an ovulation syringe (Ovitrelle) to be sure of the timing.

One straw is used per treatment. The straw is thawed, and the sperm sample is then inserted into the uterine cavity with a catheter. This makes it easier for the sperm to fertilize the egg.

If the woman lacks ovulation or has a maturation problem, hormone stimulation can ensure the necessary ovulation. If there are infertility causes in the woman, it may also be relevant to combine sperm donation with IVF treatment (IVFD)
